COVID-19 PRE-SCREEN QUESTIONS

I will contact you 24 hours before your treatment and ask you to rearrange your appointment if you would answer yes to any of these questions.

- Since booking your treatment have you or someone in your household had/ been diagnosed with Covid-19?

- Are you or anyone in your household awaiting the results of a Covid-19 test?

- In the last 10 days have you or a household member:

  • developed a new, continuous dry cough?

  • had a temperature/fever of 38 or above? (see NHS guidance on fever)

  • experienced a loss of taste or smell?

  • had difficulty breathing?

  • had close contact with someone with suspected or confirmed Covid-19?

  • been isolating or told to isolate?
